一般信息、重量和量度········································································· 11-1 1 环境、安全与健康管理 A 管理体系 1 环境、安全与健康方针 合约商应建立并保持环境、安全与健康方针,形成文件、 加以执行、并且对全体雇员进行通报。这一承诺应包括: a 合约商本身的活动与产品、服务的性质、规模和影响 相适应; b 对持续改进和污染防止的承诺; c 对遵守与之适用的有关的环境、安全与健康的法律法 规以及其他要求的承诺; d 建立和检查环境、安全与健康的目标和指标的框架; e 可为公众所获取。 2 环境、安全与健康因素 a 本节的目的是要定义合约商识别与其活动、产品或者 服务的相关的重大的环境、安全与健康因素过程和步 骤; b 这一过程应考虑成本,执行时间和数据来源的可靠 性; c 已用作程序或其他类似目的信息适用于此程序。 d 本节应说明该组织可能对所考虑到的环境、安全与健 康因素可进行控制的程度。 nike 1-2 ESH 手册 e 在制定环境、安全与健康因素/影响的识别程序的文件 时,应说明下列内容: 1) 在判断重要性时所采用的标准; 2) 需要对其进行评估的所应用的工艺流程和操作 (即工艺流程图); 3) 因素/影响识别责任的界定; 4) 考虑在正常和异常运行条件下的因素/影响; 5) 如何对新的或者计划执行的活动和工艺过程进行 评估。 3 法律法规及其他要求 a 本节的目的是确定识别和获取合约商所需遵守的法律 法规及其他要求过程; b 本节还应说明如何评估合约商对所需遵守的法律法规 及其他要求的符合情况; c 需要注意的问题: 1) 合约商应特别列明适用于合约商的环境、安全与 健康的法律法规以及相关要求(如法律法规清单 及负责更新人); 2) 合约商应明确由谁负责对草稿和最终法律法规及 其他要求进行评估和分析,以及如何记录其适用 性(例如程序参考); 环境、安全与健康管理 1-3 3) 合约商程序还应说明如何把这些要求及其变更传 达给员工及管理层。 4 目标和指标 a 合约商的目的是确定合约商建立环境、安全与健康的 目标和指标的过程和步骤; b 合约商应清晰地规定环境、安全与健康因素/影响,环 境、安全与健康方针,以及合约商对环境方面所作的 承诺与目标指标之间的关系。 c 需要注意的问题: 1) 合约商应特别说明如何建立和批准目标/指标, 如何记录、评估、审核和修改; 2) 合约商在建立与评审 ESH 目标时,应考虑法律 与其他要求,其自身的重大 ESH 因素,可选择 的技术方案,财务、运行和经营要求,以及相关 方观点。 5 环境、安全与健康管理方案 a 本节目的是明确合约商将要用旨在实现其 ESH 目标和 指标的管理方案; b 需要注意的问题: 1-4 ESH 手册 nike 1) 合约商应规定各级职能部门和管理层对达到目标 和指标所承担的责任; 2) 合约商还应包括达到这些目标和指标的办法和时 间表。 6 机构和职责 a 本节的目的是要确定合约商为了方便进行有效的环 境、安全与健康管理体系而建立的各个部门的作用、 职责和权限; b 合约商应清晰地界定贯彻执行管理体系的职责; c 合约商应指定资深管理者代表,明确其在下列方面的 作用、职责和权限: 1) 确保管理体系得到建立、实施和保持; 2) 向最高管理层报告管理体系的执行情况和效果。 合约商/雇主还应论证 ESH 管理体系所需必要资 源(人力、财力及专项技能)的提供及充分性。 7 培训、意识和能力 a 本节的目的是确定合约商如何识别培训需求以及贯彻 培训计划; b 培训计划应确保其工作内容有可能对环境、安全、健 康产生造成显著的影响的员工了解其作用和职责,确 保他们胜任工作,满足管理体系中所规定的要求; 一般性的健康要求 9-27 g 应由指定的工作人员给予药物。 h 指定的工作人员应确保药物从贴有标签的药房容器 中取出,标签上填写有孩子姓名、药物名称、剂量、 用药时间间隔、医师姓名和处方日期。应认定标签是 医生给出的指令。 i 应在孩子的卷宗里记录是谁给的药物以及给药的日 期和时间。 10 取样设备/装置指南 A 3M 有机物蒸气监测器(OVM) 这些扩散式监测器是专门用于以标准时间间隔测量平均浓度的取 样装置。取样时间将随污染物浓度和环境条件(例如湿度)而变 化。有关取样时间方面的完整细节,请参考本文档的最后一页。 本监测器既可用作个人监测器,也可用作区域监测器。用作个人 监测器时,戴在接触潜在危险环境的个人的呼吸区。当用作区域 监测器时,挂在废气运动受到限制的房间的墙上、角落、桌面或 其它区域。 图1 图2 图4 图3 图5 10-2 ESH 手册 1 nike 空白的准备 良好的工业卫生作法表明,每套样品包括一个空白监测 器可检查样品是否有任何污染。空白是一个监测器,它 已按照与样品监测器一样的方式处理,但还未暴露于被 测大气中。每十个样品至少提交一个空白。 2 取样说明 a 从容器上卸下监测器,将时间记录于监测器的后部 和相应的数据表上。保留容器。 b 在监测之前,将下列信息记录于您的数据表上:1) 监测器序列号(每台监测器有一个四位的数字和字母 );2)取样日期;3)雇员或区域 I.D.(标识符); 4)温度和相对湿度(如果知道的话);5)需分析的 化合物。 c 将日期、雇员或区域 I.D.和取样开始时间记录于监测 器标签上(图 1)。不得除去白色薄膜和塑料环。 d 监测器既可用作区域监测器,也可用作个人监测器 。对于个人取样器,将监测器放于雇员呼吸区的附近 (图 2)。把鳄鱼夹附加于衬衣的衣领上,露出白色 的薄膜侧(在外面),将监测器放于工人身上。当用 作区域监测器时,挂在废气运动受到限制的房间的墙 上、角落、桌面或其它区域。 e 当取下监测器时,将时间记录于监测器上以及数据 表上。在数据表上计算取样的总时间。如果取样周期 跨午餐时间,则从监测器放置和取下之间的总时间中 减去午餐时间。 取样设备/装置指南 103 f 在取样周期结束之后,从监测器上卸下塑料环的白 色薄膜(图 3)。合上顶盖和底盖,将其放回到罐 中,用透明胶带使其固定。现在监测器已准备好,即 可装运。注:主体部分和副体部分应具有相同的识别 号。 g 将监测器返回到罐中,用所提供的塑料盖密闭。将 各监测器牢固地装入一口箱子中。在可能的情况下, 采取冷包装,以进一步确保样品的良好保存。 h 将样品“Next Day Air”送到实验室进行分析。 注:当在相对湿度较高的情况下取样时,某些化合物[例如丙酮、 MEK(甲基乙基酮、丁酮)、醋酸乙酯烯酯等]可能显示出回收 率降低。致冷和/或加速有助于确保得到准确的结果。 将任何取样材料或设备放置于工人身上时,应采取自愿的原则。 应向工人说明,让其从事正常的工作且不要以任何方式改变其日 常工作。如果需要的话,则午饭期间监测器可保持打开状态。 为了确保监测器仍然放置正确且未损坏或被材料或衣服盖住,在 取样期间应定期检查监测器。 nike 10-4 ESH 手册 在多数情况下,被动取样“有机物蒸气监测器”(OVM)比主动 的方法更好。这一部分是以 3M 有机物蒸气监测器指南为基础的 。 每种化合物的脱附溶剂列于下表中。脱附溶剂允许个人能够确定 需取样物质同时可以在同一有机物蒸气监测器上取样多个化合物 。在同一 OVM 上可同时对脱附溶剂相同的两种或更多种化合物 进行取样。对于被取样的化合物,遵守最短和最长接触时间是至 关重要的。下表就规定这方面的信息。 *留有阴影的部分表示在鞋袜厂、制衣厂和设备制造厂取样的最 典型的化学品。 表 10 – 1:脱附溶剂 化合物 丙酮 苯 100 85 50 4 400 0.1 200 50 100 — 20 200 100 25 / 10 350 100 取样设备/装置指南 105 B 声级计使用指南 1 监测设备 a QuestTM 2400 的 2 型声级计将主要用于量化连续或间 断的噪声接触测量。操作员应记录所测得的声级。声 级计工具箱将包括一个校准仪、一套校准工具、一块 挡风板、一根碗带和两块 9 伏的碱性电池。每天取样 之前和取样之后应进行校准。不得用其它类别的校准 仪替代。QuestTM 2400 和校准仪应每年送回到经供应 商授权之前的维修和校准处。建议用户在使用之前阅 读 Quest 说明书和本指南,熟悉设备。 b QuestTM 2400 声级计禁止用于易燃大气中或者用于易 燃物质开敞容器的附近。 c 校准 QuestTM 2400 声级计: 1) 将频率加权开关滑到“A”。 2) 将量程开关滑到高(70 – 140 dBA)。 3) 将响应开关滑到“Slow”(慢)。 4) 将显示模式滑到“Run”(运行)。 5) 打开声级计的电源。LCD(液晶显示器)应显 示电池的状态和当前的声级。 6) 如果显示低电池状态,则关闭电源。打开电池 盖,丢弃旧电池。将两块新的 9 伏碱性电池滑 入电池盖里标签所示的空间中。新电池应能够 连续使用约 25 小时。建议记录下每块电池的日 期。 10-6 ESH 手册 nike 7) 打开电源。显示器应显示足够的电力(可完成 计划的测量)和当前的声级。 8) 将校准仪按到银白色的感应器上,直到完全插 入为止。当前的声级读数应比较低。(校准仪 上应有授权单位的校准日期,而且应是在一年 之内)。 9) 打开校准仪。声级应上升到校准仪源级。将适 当的声级记录于校准仪标签上。 10) 如果声级计的读数与校准仪源级标签一样,则 声级计已完全校准。 11) 如果声级计未校准,使用校准工具调节声级计 上的校准螺钉,直到其与校准仪源级标签相匹 配为止。如果声级计不能校准,则在校准仪中 安装一块新的电池。从第 8)步开始完成校准。 如果声级计还不能校准,则将其返回到授权的 维修和校准单位。 12) 关闭校准仪并从感应器上卸下。声级计已校准 。在你的笔记中记录校准已完成,包括日期、 时间和姓名。 13) 在一天的监测完成时,执行校准的第 7)步到第 12)步。 取样设备/装置指南 107 d 使用 QuestTM 2400 声级计: 1) 确保在使用之前已进行适当的校准。 2) 将碗带连接到声级计和您的碗上,以防止掉落 时损坏。 3) 在监测时不要谈话或制造噪声。所测得的所有 声级都应来自于工作环境。 4) 风或废气速度高可能在感应器周围形成啸声或 失真,这可能产生取样数据误差。在取样期间 ,在废气速度高的地方,挡风板应安装到感应 器上。如果采用了挡风板,则记于文件中。 5) 声级计应用您的臂握住,远离您的身体,放于 雇员耳朵高度附近。在有就座雇员的区域,应 在就座的雇员耳朵高度和站立的雇员耳朵高度 取样。如果没有雇员在场,高于地面约 5 英尺 (1.5 米)握住声级计。与雇员的耳朵相似,感 应器应指向侧面,大致垂直于雇员站立的方向 。记录取样高度和位置。 6) 从噪声源周围不同的作业区域监测并记录声级 。(例如,对于产品磨光机来说,在装货区、 控制盘和卸货区的附近监测。) 7) 应监测并记录涉及到噪声源的每种不同雇员。 (例如,对于产品磨光机来说,装货员、操作 员和维修人员可能有不同的噪声接触级别。) 10-8 ESH 手册 nike 8) 有些声级波动是常见的。在 10 秒钟之后,记录 每一位置的估计平均声级或中心趋势声级。如 果波动大于 3 dBA,则记录最小、最大和估计 的平均声级。 9) 也可通过将显示器开关滑到“Max Hold”(最 大)模式测定最大声级。在下一次测量之前复 位到“Run”(运行)模式。 10) 记录时间、日期、准确位置、操作中的设备和 速率。记录操作、任务或操作名称、房间或区 域的符号、在场雇员的数目、任务或接触噪声 源的持续时间、声级计的本体、所测得的声级 、完成测量的人员的姓名及其他适用的信息。 11) 噪声测量计可由雇员佩戴,以记录整个工作周 期中的各种噪声接触情况。噪声测量计记录并 计算工作日每天的噪声剂量。噪声测量计应校 准,A 加权、慢速响应、80 dBA 下限位、5 dBA 声级和 90 dBA 处的标准等的所有设置都应 完成。感应器应附于衣领上,离耳朵约 6 英寸 (15.2 厘米),向上指向耳朵。 e 监测 所有设施都应系统地进行评估,看是否有高噪声源和 高噪声工作区。高于 85 dBA 的所有区域都应识别出 来并进行监测。约 1 米以外需要雇员喊叫才能听见或 明白的任何工作区应进行评估。声级计可测量特定的 操作、机器或工作区。为了测定由单一机器引起的噪 声,可关闭邻近的设备。一项简单的计划应包括下列 各点: 取样设备/装置指南 109 1) 应由一个熟悉厂区的人员在厂区周围巡查,经过 所有工作区和维修区,识别出潜在的高噪声位置 。编制详细的噪声源和高噪声区的清单和图。应 列出各班的操作时间和预定的维修活动。 2) 查找所有噪声源附近的雇员工作区。记录他们使 用特定设备工作的时间长度以及在高噪声区的工 作持续时间。 3) 对每个已识别的工作区或噪声源进行声级测量。 记录每台邻近设备的声级和状态(开或关、全速 或半速、生产率)。 4) 雇员接触的声级高于时间加权平均值 90 dBA 以 上的所有设备应进行评估,看是否需要工程控制 、隔音、增加吸声材料、用更安静的设备替换等 。 5) 如果有噪声测量计,噪声测量计可由雇员佩戴, 以记录接触的持续时间和接触程度。将噪声测量 计的记录与测得的声级和估计的接触时间进行比 较。应再次监测二次测量值超过 3 dBA 的区域。 10-10 ESH 手册 nike 6) 对大于 85 dBA 的所有噪声源区域和所有雇员工 作区进行概括。将声级标于平面布置图上,工作
