1 PROBLEM STATEMENT The necessity of the dissertation People's Public Security is an armed force that serves as an advisory unit for the Party and the Government on defending National security and protecting social order and safety, fighting and preventing operation conspiracies of hostile forces, criminals and law violations regarding National security and social order and safety In order to meet the demands as well as missions in the new situation, the Public Security Minister has decided to establish the High Command of Mobile Police Force (K20) with the mission to cooperate with armed forces to suppress and prevent acts of sabotage, riot instigation, terrorism, hijacking, hostage abduction, and pursue armed gangs and organized crimes During the mission enforcement, the number of public security officers and men suffering from injuries and deaths has a tendency to increase for many reasons, one of which is that the task of guaranteeing health for public security officers and men in general and for mobile police force and special task force in particular still has many improprieties and has yet to meet the requirements of the missions, especially the task of guaranteeing combat health nowadays Stemming from demands in the reality of working and fighting of K20, the proposed demand for the research and establishment of medicine list, medical equipment, combat medicine basic load and methods of guaranteeing medical materials for K20 in this period is an urgent and imperative demand Objectives of the dissertation: To describe the real situation of the disease model and the task of guaranteeing medical materials for K20 (2006-2010) To build a medical equipment and medicine list for K20, combat medicine basic loads and methods of packaging the basic loads for the Special Task Force To initially suggest and assess methods of guaranteeing medical materials for K20 Contents of the dissertation In order to accomplish the set objectives, the dissertation consists of the following contents: + Surveying the reality of the disease model and the task of guaranteeing medical materials of K20, namely: - Analyzing the reality of medical examination and treatment, the ratio of wounded soldiers, sick soldiers, the wound structure of K20 in forms of combat, proposing the disease model for K20 - Surveying the facilities, medicines, medical equipment, combat basic loads, medical materials of K20 to serve as a foundation of the proposal for the establishment of methods of guaranteeing medical materials for K20 + Building adequate and fundamental contents for the list of medicines and medical equipment for K20 + Building simultaneously the combat medicine basic loads for the Special Task Force (basic loads for Battalions, Companies and Individuals) Researching methods of packaging the combat medicine basic loads with materials that are suitable, safe and convenient for modes of transportation + Suggesting, surveying and assessing methods of guaranteeing medical materials for K20 New contributions of the dissertation On the basis of surveying the reality of the task of guaranteeing medicines, medical equipment, basic loads and medical examination and treatment (2006-2010), the dissertation is the first to propose the disease model of K20 showing that the group of diseases caused by injuries accounts for a large percentage: 19.15%, the proportion of wounded soldiers in forms of fighting (fighting riots and protests: 4.83%, attacking criminals: 1.83%), in marching and training: 1.43%, the proportion of wounded soldiers in term of injury level (severe level 7.5%, medium level 12.5%, minor level 80%), which serves as a scientific foundation for the research and establishment of the list of medicines and medical equipment and combat medicine basic loads for K20 For the first time, the research builds a medicine list for K20 with 209 medicines arranged according to names of active elements of 22 groups of pharmacological effects, an adequate list of medical equipment including groups of 376 pieces of equipment, and a list of 25 fundamental pieces of medical equipment for K20 that meet the mission requirements of K20 The research builds simultaneously combat medicine basic loads: Battalion, Company and Individual Basic Loads (KB, KC, KN Basic Loads) for the Special Task Force based on analyzing scientifically obtained and surveyed data as well as real practice of K20 Research results have been exploited for use in K20 and have been initially highly considered by the unit as a study of high scientific value and practical significance The research focuses on methods of guaranteeing medical equipment for K20 based on principles of sufficient, comprehensive and imperative supply, and at the same time creates flexibility and initiative for the unit Guarantee methods have been implemented and applied in K20 and have been confirmed to have many advantages that are suitable for the market economy Dissertation structure The dissertation consists of 145 pages, 36 tables, charts, pictures, 125 reference materials The outline of the dissertation includes: - Medical records of wounded soldiers and sick soldiers of K20 infirmary and medical units directly under K20 - List of medicines, medical equipment, medicine basic loads used in the world and in Vietnam - Medical staff, Special Force officers and men of K20 - Experts on military medical supply from Ministry of Public Security and Ministry of National Defence 2.1.2 Research materials: - Documents, materials and regulation documents of the Party, the Government specifying missions of security and defence - Documents, materials, regulation documents specifying personnel organization, medical standards and norms, professional regulations - Didactic materials on guaranteeing ordnance task in the Military, on organizing command of military health and military medical ordnance - Documents introducing medical equipment in order to select suitable equipment for the treatment mission, for the medical science and technology development trend and the competency of the industry - Investigating materials obtained from reports - Materials serving the research on packaging basic loads of: medicines, medical tools, packaged materials 2.1.3 Research location and time frame: + Research location: K20 infirmary; Special Force Battalion: No 1, 2, 3; Institute of Pharmacology Training and Research (K85) – Military Medical University; Health Department, functional departments of Health Department + Research time frame: In the period 2011- 2013, the research was split into phases: - Phase 1: Researching the reality and carrying out retrospective research on data and figures within data stored from 2006-2010 - Phase 2: Building the list and proposing assessments through experimental maneuver carried out at Special Force Battalion No (Hanoi) on 12/2012 2.2 RESEARCH METHODOLOGY 2.2.1 Research design: Retrospective research through analyzing secondary data, combined with cross-sectional descriptive method, applied in experimental maneuver implementation and qualitative and quantitative researches in order to complement and complete the research 2.2.2 Sample size and sampling method: - The research uses purposive sampling Analyzing the reality of medical examination and treatment, medical equipment and medicine list, combat medicine basic loads and the task of guaranteeing medical materials of K20 - Sample size for sociological survey: 50 medical officers of K20 working at the infirmary and health units of Special Force Battalions; 75 Special Police of K20; 25 medical experts and specialists of Hospitals and Academies of Public Security and Military 2.2.3 Research methodology: - Retrospective descriptive method - Experimental maneuver method - Sociological method 2.2.4 Data collection methods - Questionnaires - Interviews - Seminars - Experts’ opinions - Reports - Practical observations 2.2.5 Data processing Research data are processed in Excel 2.2.6 Research scope: This research is only a research on establishing a medicine list, medical equipment list, suggesting methods of guaranteeing medical materials for K20, combat medicine basic loads (Battalion, Company, Individual) for Special Police of K20 Chapter RESEARCH RESULTS 3.1 SURVEY ON THE REALITY OF THE DISEASE MODEL AND THE TASK OF GUARANTEEING MEDICAL MATERIALS OF K20 3.1.1 Infrastructure, human resources and qualifications of health: - Results show that the infirmary has a floor area of 350m 2, a health examination room, a treatment room, a laboratory, a fundamental medicine storage guaranteeing medical examination and treatment at K20 infirmary - Regarding human resources, there are 60 medical officers (9 doctors, 25 physicians, pharmacists, 17 nurses) assigned in the Infirmary and subordinate units, yet they are unsuitable At the time, Regiments namely E20, E22, E23, E24 and Battalions namely DN1, DN2 have Doctors, E21, E25, DN3 have no Doctor 3.1.2 Disease model at K20 infirmary (2006-2010): - Survey results at infirmary show that: Acute diseases not caused by injuries 37.40%; Infection: 25.5%; Injury: 19.15%; Chronic diseases: 10.30%; Other diseases: 7.65% The most common disease was influenza - In 2010, the most common disease was influenza accounting for 50%; Pharyngitis 28.10%; Gastric syndrome 10.71%; Injuries caused during marching or training accounted for 1.43% 10 3.1.3 Injury structure and proportion of wounded soldiers - Injury structure according to wounded parts: Flesh wounds: 34.87%; Lower limb trauma: 28.53%; Upper limb trauma: 19.02%; Chest and abdominal trauma: 10.08%; Head-Face-Neck trauma: 6.63%; Traumatic brain injury: 6.34% - The proportion of wounded soldiers according to the degree of the wounds: severe 7.5%, medium 12.5%, minor 80.0% The proportion of wounded soldiers according to forms of fighting: Fighting riots and protests: 4.83%; Attacking criminals: 1.83%; During training, marching, working: 1.43%; According to antipersonnel weapons: Lethal weapons account for 63.6% 3.1.4 Results of survey on the reality of medical equipment, list of medicines and combat basic loads of K20 (2006-2010): - Medical equipment for emergency treatment and health examination at K20 infirmary in 2006 had 20 items, and in 2010 had 29 items - List of medicines used at K20 infirmary has 87 items classified into 16 groups according to their pharmacological effects (Antibiotics: 28.2% Cardiovascular and metabolism disorder 11.4% Minerals and Vitamins 10.1% Painkiller, febrifuge, digestion medicines 6% Diuretics and antidotes 1.3%) - Essential medicines account for: 90.7% 3.2 BUILDING MEDICAL EQUIPMENT AND MEDICINE LIST FOR K20 AND COMBAT MEDICINE BASIC LOADS FOR THE SPECIAL TASK FORCE 15 sufficient types of medicines, bandages, tools, including straps to attach to the belt, closing and opening easily and conveniently for soldiers in use, basic load weight: 250g Basic load marking: DEPARTMENT OF HEALTH MINISTRY OF PUBLIC SECURITY KN BASIC LOADS 3.2.3.6 Results of opinion collection + KB basic loads: - Opinions of medical officers about 88% of the votes agreed with the draft; 12% suggested increasing the quantity and medicines After group discussion, a consensus was reached not to increase the quantity, not to supplement skin medicines, digestion medicines and Prednisolone, Gentamicin and Spiramycin because there are already Povidone, Biseptol, Methylprednisolone, Rodogyl in the list Agreed to supplement Nifedipine 10mg, 20 tablets - Opinions of medical experts: 3/25 votes suggested supplement and replacement as follows: Replace Inox splints with bone-fixing soft splints; Cord Tourniquet with self-use Tourniquet; Supplement Petrov needle to prevent valve pneumothorax We obtained suggestions for the list of KB basic loads including 61 items 33 medicines, 28 tools, bandages and gauze + KC basic loads: - Opinions of medical officers about 24% of the votes suggested increasing the quantity Namely: 07 opinions suggested retaining Emergency Alcohol; 01 opinion suggested retaining both Silvirin and 16 Panthenol; 01 opinion suggested increasing the quantity of painkilling aerosol medicines 03 boxes and Panthenol 02 boxes After researching and obtaining contribution opinions, we agreed with suggestions to supplement Phat Linh Truong Son Medicated Oil 2ml to replace with the quantity of 20 boxes - Opinions of medical experts: 1/25 vote suggested supplement as follows: replace rubber Tourniquet with self-use Tourniquet, supplement bone-fixing soft splints - Obtaining opinions and adjusting KC basic loads including: 35 items 21 medicines, 14 tools, bandages and gauze + KN basic loads: - Opinions of medical experts: 22% of the votes suggested supplement, namely: 07 opinions suggested retaining sterile gloves, 02 opinions suggested omitting Morphine, 03 opinions suggested supplementing diarrhea medicines, skin medicines, allergy and insect bite medicines - Opinions of Special Police: 30 opinions suggested retaining sterile gloves, 03 opinions suggested omitting Morphine, 04 opinions suggested omitting sterile adhesive skin closures, 03 opinions suggested supplementing and increasing NaCl 0.9% from 01 vial to 03 vials, adhesive tapes with sterile gauze to 03 pads - Opinions of medical experts: 2/25 votes suggested: Omitting Loratadine, Paracetamol, saline 0.9%, sterile gloves; Replacing Povidone 10% from 20ml to 50ml; Supplementing self-use 17 Tourniquet We obtained opinions and adjusted KN basic loads including 13 items medicines, tools, bandages and gauze 3.3 SUGGESTIONS, SURVEY AND ASSESSMENT OF METHODS OF GUARANTEEING MEDICAL MATERIALS FOR K20 3.3.1 Principles and foundations of proposals Methods of guaranteeing medical materials for K20 not only guarantee medicines, medical materials for regular tasks, fighting readiness and fighting, but also guarantee imminent requirements, and also satisfy requirements for long-term development and are built based on principles: standards and norms of types of materials for each subject, State Budget criteria, organization of personnel and missions of the unit, combination of military and civic medical services in the area 3.3.2 Suggestions for methods of guaranteeing medical materials for K20 + The general model of methods of guaranteeing medical materials for K20 follows a closed process including a series of activities: Planning - Supply - Storage - Distribution - Consumption + Contents of the methods include: Planning task; Supply organization and source production task; Distribution task; Storage task; Consumption management 3.3.3 Results of survey and assessment of methods of guaranteeing medical materials for K20 18 + Collecting opinions from medical officers about the guarantee methods: Results 56% of the opinions agreed with the methods of guaranteeing medical materials for K20 that combine in kind and in cash + Supplying to guarantee medicines and medical equipment, survey results are as follows: - Department of Health: guarantees 10-12% of medicines and consumed chemicals and materials; 88% medical equipment - K20: guarantees 68-72% of medicines and consumed chemicals and materials; 10 % medical equipment - Units under K20: Guarantee 8-22% of medicines and consumed chemicals and materials, 12% medical tools + Basic load storage: Survey results: Department of Health: 50 -74%; K20 infirmary: 38 -50%; Medical units of Battalion: 8-12% + At the seminar, collecting opinions of experts about methods of guaranteeing medical materials for K20 yielded the following results: - General model: 100% of opinions from experts agreed with the suggested model - Contents of the methods: Planning task, supply organization task, distribution, consumption, guaranteeing regular missions and fighting: 25/25 opinions (100%) agreed with the draft Particularly for the storage task, 01 opinion suggested increasing the basic load storage at the Battalion 19 Chapter DISCUSSION 4.1 SURVEY ON THE REALITY OF THE DISEASE MODEL AND THE TASK OF GUARANTEEING MEDICAL MATERIALS FOR K20 (2006-2010) + Disease model, the proportion of wounded soldiers in action - Regarding disease model: According to research results at K20 infirmary, it is shown that the group of diseases caused by injuries has high proportion: 19.15%; (public security 9.21%, community 8.6%) which requires medical services at K20 to focus on guaranteeing and performing intensive treatment for injury emergency In forms of fighting, the proportion of wounded soldiers - In forms of fighting, the proportion of wounded soldiers from fighting riots, protests and rebels: 4.83%; Attacking criminals: 1.83%; Injured from training and marching: 1.43% In comparison with military medical mission summary data in the anti-American resistance war in the Southern battlefield, it shows the average proportion of wounded soldiers in the campaign: 11.46% Therefore, the proportion of wounded soldiers in forms of fighting of Public Security is usually lower than that in the Army - The proportion of wounded soldiers in term of injury severity: severe 7.5%, medium 12.5%, minor 80.0% In comparison 20 with military medical mission summary data in the Southern battlefield, it shows that: severe 15.34%, medium 29.38%, minor 55.28% Therefore, when Public Security officers are wounded, they mainly suffer from minor injuries - According to wounded part: Flesh wounds 34.87%; Lower limb fractures 28.53%; Upper limb fractures 19.02%; Chest and abdominal trauma 10.80%; Head face neck 6.62%; Traumatic brain injuries 6.34% The comparison with the Army shows that: For both Public Security and the Military, the most commonly wounded part is the flesh (34.87/65.23) - Traumatizing factors: lethal weapons account for a high rate of injury and death 63.6%, in which death accounts for 27.3% Primitive weapons such as: thick bamboo stick, knife, falchion, brick, rock account for 36.4% rate of injury + The reality of material facilities, human resources and medical qualifications of K20: - Regarding material facilities, K20 infirmary basically guarantees sufficient area for health examination and emergency treatment - Regarding medical equipment, the infirmary still lacks some important basic pieces of equipment to serve trauma emergency - Regarding the medicine list of 87 medicines, it is still unable to meet the requirements for outdoor emergency and treatment 21 Survey results of the reality of material facilities, medical equipment, human resources, expenditures show that methods of guaranteeing medical materials of K20 combine guarantees in kind and in cash, but they have yet to meet the requirements for the missions and combats of K20 in the current period 4.2 METHODS OF BUILDING MEDICAL EQUIPMENT AND MEDICINE LIST FOR K20 AND COMBAT MEDICINE BASIC LOADS FOR THE SPECIAL TASK FORCE 4.2.1 Regarding methods of calculation and establishment of list contents and basic load packaging method Particularly we have built: - Adequate and fundamental list of medicines and medical equipment - List of combat medicine basic loads for the Special Task Force - Methods of packaging combat medicine basic loads 4.2.2 Regarding the implementation of opinion collection: Divided into phases: - Phase 1: Collecting opinions from direct combat units of K20 - Phase 2: Collecting opinions from medical officers of K20 and Special Force Police who are the direct consumers of the basic loads 22 - Phase 3: Collecting opinions from leading experts and officers in military medical ordnance of Ministry of National Defence, Ministry of Public Security 4.2.3 Regarding results of contribution opinion collection: - Most of the opinions suggested increasing the quantity The number of votes with opinions and the number of opinions in one vote decreased - Some pieces of equipment and medicines, although they have only 01 opinion, are very practical such as: Supplementing antihypertensive medicines, Self-use Tourniquet to replace rubber Tourniquet, Soft splints to replace Inox splints, Petrov needle in KB basic loads, Sterile gloves in KN basic loads - The number of new additions is not high, especially in the list of medical equipment, list of main medicines as well as combat medicine basic loads 4.2.4 Regarding contents of established list 4.2.4.1 Establishing the list of medical equipment - The draft list proposed for the collection of opinions from medical officers K20 and experts includes 376 pieces of equipment agreed with by medical officers of K20 as well as medical experts - The selection of 25 lists of most fundamental medical equipment so that research results have practical significance and suitable application for direct combat units such as K20 infirmary - Expert opinion collection seminar on lists of fundamental medical equipment: 25/25 opinions (100%) agreed with suggestions 23 4.2.4.2 Establishing medicine list used for K20 - The initial draft list proposed for the collection of opinions from medical officers K20 includes 409 medicines - After collecting opinions from medical officers K20 and medical officers of direct combat units, we made some adjustments to decrease to 209 medicines - Collecting opinions for the third time from experts: 25/25 opinions (100%) agreed with the suggestions 4.2.4.3 List of combat medicine basic loads A List of combat medicine basic loads for Battalion: KB BASIC LOADS - List of KB basic loads initially proposed includes 56 items of 15 groups (11 groups of medicines and 04 groups of tools, bandages) - After collecting opinions for the second time from medical officers, we built the list of KB basic loads including 60 items, 32 medicines, 10 bandages and gauze, 04 syringe and surgical thread items, 09 minor operation tools and 05 other tools - Collecting opinions from medical officers for the third time and experts reached a consensus over the proposed list and suggested replacing Inox splints with bone-fixing soft splints, rubber Tourniquet with self-use Tourniquet, and supplementing Petrov needle - As a result, KB basic loads include 61 items, 33 medicines, 28 tools, bandages and gauze 24 KB basic loads that we built not only serve Special Force Police but also serve, complement, support Y base number in the defence area performing the task of preventing and fighting riots, protests, overturns B List of combat medicine basic loads for Company: KC BASIC LOADS - List of KC basic loads initially proposed includes 33 items 20 medicines, 13 medical tools, bandages, classified into 05 groups of medicines (Antibiotics, hemostatics, painkillers febrifuge tranquillizers muscle relaxants, antiseptics, other medicines) and 01 groups of medical tools - After collecting opinions from medical officers K20 and from experts, we received suggestions to replace rubber Tourniquet with self-use Tourniquet and supplementing bone-fixing soft splints - Therefore, KC basic loads include 35 items 21 medicines, 14 tools, bandages and gauze KC basic loads can supersede Medicine Bag of physicians, can be equipped for teams and squads of traffic police to perform first aid in traffic accidents, target protection commanding positions C List of combat medicine basic loads for Individual: KN BASIC LOADS - List of KN basic loads initially proposed includes 11 items 04 medicines, 07 tools, bandages and gauze - After studying the T81 base number, D84 base number Chemical antidote base number used for Infantry proposed a list of 13 items medicines, medical tools, bandages 25 - Collected opinions from experts suggested omitting Loratadine, replacing rubber Tourniquet with self-use Tourniquet, retaining Sterile gloves As a result, list of KN basic loads includes 13 items 07 medicines, 06 tools, bandages and gauze During the establishment, we especially paid great attention to shock preventive medicines, toxic preventive medicines, tools for cardiopulmonary resuscitation, rubber gloves Compared to the tactic rescue medical emergency bag of New Zealand used in the police force with 11 items, KN basic loads display more benefits Research results of the establishment of KB, KC, KN basic loads for the Special Task Force of K20, Ministry of Public Security make great scientific contributions of practical significance 4.3 METHODS OF GUARANTEEING MEDICAL MATERIALS FOR K20 The task of guaranteeing medical materials serving the training tasks, fighting readiness and combat is a mission of Department of Health and health units of K20 Guaranteeing medical materials in Public Security as well as Military is determined based on military medical standards, criteria and is distributed to wounded soldiers, sick soldiers for their treatment until they are stable again and get back to their units for further duty Methods of guaranteeing medical materials for K20 are built in accordance with a closed process with a synchronous combination of methods in phases such as planning, supply, source production, storage, distribution and consumption 26 management in order to meet the demands for taking care of and enhancing health for Public Security officers and men, guaranteeing necessary medicines and medical equipment for units to serve their tasks and combats Methods of guaranteeing medical materials for K20 are built based on acquiring previous medical guarantee methods that are to guarantee medical materials both in cash and in kind Department of Health guarantees 100% medical equipment, 50% combat medicine basic loads for the health of K20 Medicines, chemicals will be added under sudden circumstances such as protests, riots, disasters, catastrophes K20 unit is subsidized by the Department of Finance with 100% expenditures according to their standards and norms in order to actively purchase medicines and normal medical materials, package combat basic loads by the principle of bulk packaging Using bulk packaging in order to recover basic loads that have only a few items used increases the stored quantity and creates the initiative in medical materials, basic loads for Special Force Regiments, Battalions In combat medicine basic loads, KN basic load has Morphine sulfate 10 mg packaged in the basic load which is an addictive medicine managed at health units When Special Force Police are engaged in combat, they are equipped with this medicine in only a small dose and they can only use the medicine when ordered by the commander Besides, the basic loads are also assigned to individuals only before engaging into combat After the mission has been 27 accomplished, the police get back to the units and an inventory will be carried out to redetermine the list and manage according to regulations over addictive medicines So it is not too worrying about the medicine abuse during management Regarding previous basic load guarantees, Department of Health established, packaged and supplied to forces including K20, at which time K20 was only a recipient unit, which is reasonable However, when the combat medicine basic loads for K20 are established, 50% will be handed over to the unit for active basic load packaging, storage, and preservation in Regiments, Battalions At K20 infirmary, new methods will be implemented in forms of medical supply with basic loads in combination with bulk packaging, which will save materials, budgets, and create initiative in packaging when needed, combined with storage following the line High Command -
