THÔNG TIN LUẬN ÁN TIẾN SĨ ĐƯA LÊN MẠNG Tên đề tài luận án: Nghiên cứu chất lượng sống bệnh nhân sau mổ nội soi cắt túi mật sỏi Mã số: 62.72.07.30 Chuyên ngành: Ngoại – Gan mật Họ tên nghiên cứu sinh: Họ tên cán hướng dẫn: HỒ THỊ DIỄM THU PGS TS TRẦN THIỆN TRUNG Tên sở đào tạo: Đại học Y Dược TP.Hồ Chí Minh TĨM TẮT NHỮNG KẾT LUẬN MỚI CỦA LUẬN ÁN: Nghiên cứu đánh giá chất lượng sống 398 bệnh nhân trước sau cắt túi mật nội soi sỏi Bệnh viện Nguyễn Trãi Bệnh viện Đại học Y Dược TP Hồ Chí Minh cho thấy: Trước mổ: phần lớn bệnh nhân có chất lượng sống bị ảnh hưởng với triệu chứng lâm sàng đau bụng, đầy bụng, ăn khó tiêu Bệnh nhân đánh giá chất lượng sống câu hỏi SF – 36 GIQLI Sau mổ tháng: phần lớn bệnh nhân có chất lượng sống cải thiện tốt chiếm 78,1 % trường hợp Sau mổ tháng: có 15 bệnh nhân có chất lượng sống bị ảnh hưởng Số bệnh nhân có chất lượng sống cải thiện tốt chiếm 96,2 % trường hợp Sau mổ tháng: bệnh nhân bị ảnh hưởng đến chất lượng sống, số bệnh nhân có chất lượng sống cải thiện tốt chiếm 99,7 % trường hợp Song kể từ tháng thứ khơng cịn bệnh nhân bị ảnh hưởng So sánh chất lượng sống bệnh nhân sau mổ so với trước mổ: nhóm bệnh nhân khơng triệu chứng lâm sàng trước mổ, sau mổ chất lượng sống có bị ảnh hưởng (chiếm tỉ lệ khơng cao 5,3 %) sau có cải thiện tốt trở bình thường sau tháng Đối với nhóm bệnh nhân có triệu chứng lâm sàng trước mổ, sau mổ chất lượng sống bị ảnh hưởng nhiều (chiếm 16,6 %) cải thiện tốt dần trở gần bình thường vịng tháng Như vậy, sỏi túi mật có triệu chứng: có định mổ chất lượng sống sau mổ có cải thiện tốt Sỏi túi mật không triệu chứng: qua nghiên cứu thấy thật >30 % bệnh nhân có chất lượng sống sau mổ không thay đổi nên định mổ nên cân nhắc, chọn lọc định mổ cẩn trọng mổ TP.Hồ Chí Minh, ngày 18 tháng 01 năm 2014 NGƯỜI HƯỚNG DẪN NGHIÊN CỨU SINH PGS.TS TRẦN THIỆN TRUNG HỒ THỊ DIỄM THU HIỆU TRƯỞNG ONLINE PhD DISSERTATION INFORMATION The PhD Dissertation Title: Research on life quality of patients after removing gallbladder with stones endoscopically Speciality: Hepatobiliary Surgery Code: 62.72.07.30 PhD candidate: Ho Thi Diem Thu Supervisor: Tran Thien Trung, PhD, Associate Professor of Surgery Academic Institute: University of Medicine and Pharmacy- Ho Chi Minh City SUMMARY OF NEW FINDINGS Research and assessment on life quality of 398 patients before and after removing their gallbladder with stones endoscopically at Nguyen Trai Hospital and Hospital of University of Medicine and Pharmacy- HCM city revealed as follows: Before operation: Most of the patients had had life quality affected by clinical symptoms, such as abdominal pain, bloating and dyspepsia The patients were assessed by answering two question sections of SF-36 and GIQLI After a month operated on their gallbladder endoscopically, most patients with improved life quality accounted for 78.1% of cases After months subsequent to the procedure, only 15 patients had life quality involved The number of patients with positive life quality accounted for 96.2% of cases After months, only one patient had his life quality affected The number of patients having good life quality accounted for 99.7% of cases However, since seventh month, there was no patient affected by the disease Comparison of life quality of patients before and after endoscopic procedure: For a group of patients without clinical symptoms prior to endoscopy, their life quality seemed to be affected (less than 5.3%); but after that it was improved well and turned to normal after months For a group of patients with clinical symptoms prior to endoscopy, their life quality seemed to be much more affected (16.6%); gradually improved and turned to normal within months In summary, the symptomatic gallbladder with stones, which was designated to remove because of quality of life after endoscopy was well improved The asymptomatic gallbladder with stones based on this research showed that actually over 30% patients who had unchanged life quality should be considered to select carefully when designated to perform endoscopy Ho Chi Minh city, January 18, 2014 PhD candidate Supervisor TRAN THIEN TRUNG, A/Prof, PhD HO THI DIEM THU Rector
